This is the Wilmette Arts Guild's Forms Page. Here you can search for any available W.A.G. forms. Click on the appropriate button below and you will be wisked away to that form. Please either print your form, fill it in and mail it to us or fill in the online form, then click the SUBMIT button at the end. Thank you for being green!
Select this form to either join or renew your membership to the Wilmette Arts Guild.
Select your level of membership, fill in the form then press SUBMIT. You will be directed to a page where you can pay your dues with a credit card. If you prefer you can mail a check.
Select this form to download, print and mail back to the Guild OR better yet, fill it in online, press SUBMIT and you will be entered into the Gallery Lottery and you will then be wisked off to a new page to pay your dues if you owe them and/or your Gallery $20 publicity fee.
Select this form to download and print at home in triplicate. When you show up for a gallery assignment or Members' Show hanging, this is the only paperwork you will need. Preparing these forms in advance speeds the time you spend hanging for your show.
